I understand that I can cancel my appointment UP TO 48 HOURS BEFORE my chosen time using the online booking system and receive a refund.

I understand that I can reschedule my appointment UP TO 24 HOURS BEFORE the chosen time using the online booking system.

I understand that if I do not attend my appointment or wish to reschedule or cancel my appointment WITHIN 24 HOURS of my chosen time, Health Adjusted reserves the right to not refund the appointment fee.
